HOW TO PREPARE FOR A SUCCESSFUL APPOINTMENT

FORMS OF IDENTIFICATION

Each signer must present to the notary at least one form of acceptable identification. Identification (satisfactory evidence) must meet all of the following criteria. It must:


  1. be current (unexpired)
  2. be state- or federally-issued
  3. contain the signer’s photograph and a physical description or signature

ACCEPTABLE FORMS OF IDENTIFICATION

  • State-issued driver's license
  • State-issued identification card
  • U. S. passport
  • Military service ID issued by the U.S. government
  • Alien registration card (green card)
  • Federally-recognized tribal government identification card

UNACCEPTABLE FORMS OF IDENTIFICATION

  • Matricula Consular Card
  • Medicare Cards
  • Student Identification
  • Gym Membership Card
  • Social Security Cards
  • Temporary Driver’s Licenses
  • Driver’s Licenses without photographs

UNACCEPTABLE NOTARY SITUATION

Signer is not present (except for Power of Attorney or Proof of Execution by a Subscribing Witness situations).

BEST PRACTICES: BEFORE APPOINTMENT

  • Do not sign document until the notary instructs you to sign.


  • All signers must be present at the time of the appointment, with the notary in attendance. 


  • All parties must be coherent and not under the influence of alcohol or sedatives. Family members and caregivers of the signer should watch that the signer is not given any such sedatives or alcohol. 


  • Additionally, please wake the signer, if sleeping, at least one hour prior to signing.
